Scott Spradlin, a licensed professional counselor and licensed master’s addiction counselor, is the co-director of Wichita DBT at NorthStar Therapy, along with his wife, Mariah Spradlin, a licensed clinical social worker and licenses clinical addiction counselor. They provide adherent comprehensive DBT and Wise Mind for the Distracted Mind: DBT for Adult ADHD, a modified DBT for adult ADHD.

Scott completed DBT intensive training with DBT founder Marsha Linehan, Ph.D., and Linda Dimeff, Ph.D., in 2000 in Seattle, Washington. He served as an adjunct professor for the College of Education and School Psychology at Wichita State University, where he taught courses on DBT and related topics, the assessment and treatment of suicidality and self-harm, and the ethical integration of religion and spirituality in psychotherapy for undergraduate and graduate students. Scott authored Don’t Let Your Emotions Run Your Life: How Dialectical Behavior Therapy Can Put You in Control (New Harbinger, 2003).

He and his wife are co-authoring a new workbook on DBT for adult ADHD scheduled for release in 2026.

To clarify, in addition to the contraindications to transdermal MHT/HRT that were discussed (history of breast cancer and complicated diabetes), there may be other relative contraindications for oral MHT, such as a history of thrombosis or migraines. These were not fully addressed due to time constraints. Importantly, transdermal estrogen is generally considered safe for individuals with a history of thromboses, and certain formulations of local (vaginal) estrogens can be used by all women without time restraint. For specific questions and concerns, consult your healthcare provider.


Episode Description

Brain fog, emotional meltdowns, and fear of dementia. Perimenopausal women with ADHD face these and myriad other challenges to brain and body due to hormonal changes during their final reproductive years. Many women, with and without ADHD, are not fully aware of the hormonal processes that start up to 10 years before menopause. During this busy time of life, many women carry great responsibility for children, aging parents, career, and family logistics. Already burdened by stress, most women assume the physical and psychiatric symptoms of perimenopause are unavoidable. At the same time, they struggle to understand and effectively treat their changing ADHD symptoms during this time of hormonal transition, which has been largely ignored by medical research to date.

All of this is true, however many researchers and clinicians continue to dismiss patients’ concerns saying that “there are no studies exploring adult or older women” or “there is no evidence to support advice on adjustment of medication or other treatment efforts for (peri)menopausal women.” It is, of course, important to base our medical advice on the best available research. But is it ethical to allow adult women with ADHD to suffer just because researchers and clinicians haven’t paid them enough attention?

We think not! That is why I’m passionate about doing the best and the most we can and to communicate the evidence that actually exists.

ADHD complicates relationships on its own, but adding rejection sensitive dysphoria (RSD) to the equation almost always amplifies the strife. RSD describes unbearable, painful feelings related to a real or a perceived rejection, and the ensuing belief that people will pull back their love and support. Cheryl Chase, Ph.D., is a licensed clinical psychologist in private practice in Independence, a suburb of Cleveland, Ohio. She specializes in the diagnostic and neuropsychological assessment of various conditions impacting children, adolescents, and young adults including ADHD, Learning Disorders, and emotional concerns. She is PSYPACT certified and able to serve clients from the 40 participating states. In addition to her clinical practice, Dr. Chase is also an accomplished international speaker, leading workshops on such timely topics as executive functioning, dyslexia/dysgraphia, and creative ways to support those who struggle in school. Finally, Dr. Chase serves as an adjunct instructor at several colleges in the Cleveland area. A cluttered, disorganized living environment is as common as it is unsettling for many people who have ADHD. Overwhelm often prevents them from even getting started on decluttering. For some people, the clutter reaches a level that compromises their ability to live safely in their homes. This is usually the result of hoarding disorder. Sometimes, it’s difficult to discern what constitutes clutter versus hoarding.

Michael Tompkins, Ph.D., will explain why the traits in ADHD contribute to excessive clutter and hoarding. He will also walk viewers through the process of managing these often-debilitating situations and employing strategies for success.

Russell Ramsay is retired from his role as professor of clinical psychology in the psychiatry department of the Perelman School of Medicine at the University of Pennsylvania. He had been co-founder and co-director of the PENN’s Adult ADHD Treatment and Research Program. He currently has a solo, completely virtual psychology practice.

Dr. Ramsay has authored numerous professional and scientific articles and book chapters. Among his books, he is author of the patient guidebook The Adult ADHD Tool Kit (with Dr. Anthony Rostain; Routledge, 2015), which has been translated into French, Spanish, Korean, and is in the process of being translated into German and Chinese.
